2. Mail the form and payment to: Tamika Hall, Administrator, PSP, 313 S. 16th Street, Philadelphia PA 19102. Form and payment must be received by September 15, 2008. Register by August 22, 2008 and receive a 5% Early Bird discount!
   

3. You will receive written confirmation that we have received your registration within one week from the date we receive it
